Reminder: Please review the Eligibility section for information on the licences you may qualify for.

  • All forms are available in PDF format. Click on the interactive form from the list below that you want to view.

  • When filling out the forms you can:
    • complete the forms online by pointing your mouse to the area that you want to fill in and click. You may move forward through the form with the TAB key, or backwards with the SHIFT + TAB key;
    • print the forms by clicking on the print icon in the Adobe menu bar and fill out any remaining sections with pen;
    • save the blank forms to your local computer by clicking on the save icon in the Adobe menu bar; and
    • submit the forms to SLGA as per the instructions provided within each form.

Bingo

Class A - Addendum Request.pdf
This form is to be used by an association bingo hall to make changes to their scheduled events.

Class A - Expense Details.pdf
A statement comparing actual vs. budgeted expenses for the month. The statement shall include an explanation of any significant variances between actual and budgeted expenses.

Class A - Event Close-Out Summary.pdf
A document which details all financial transactions related to a particular Bingo/Charitable Gaming Event in a format authorized by SLGA.

Class A Bingo-Charitable Gaming Event Log.pdf
A log which includes the revenues, prizes, expenses and proceeds generated as a result of their Bingo/Charitable Gaming Events. The Licensee shall complete the Bingo/Charitable Gaming Event Log on forms prescribed, or approved, by SLGA and submit these forms to SLGA by the last day of the following calendar month.

Class B, C, C Restricted, Small - Bingo Addendum Request.pdf
This form is to be used by a bingo licensee to make changes to any of the information that appears on their bingo licence.

Class B Bingo-Charitable Gaming Event Application.pdf
Use this application form to apply for a bingo or charitable gaming event licence, if you are a charitable or religious organization who has received approval from an association bingo hall to run events within that hall.

Class C Bingo-Breakopen Application.pdf
Use this application form to apply for a bingo licence, and if desired a breakopen licence, if you are a charitable or religious organization who will be running no more than three bingo events a week in a community bingo hall and where the prize value at an event may exceed $1,000.

Class C - Event Close-Out Summary.pdf
To be used by a Class C bingo licence holder after each bingo event.

Class C Bingo Monthly Event Log.pdf
To be used by a Class C bingo licence holder to record bingo events held during every month and submitted to SLGA by the last day of the following calendar month.

Class C Restricted Bingo Breakopen Application.pdf
Use this application form to apply for a bingo licence, and if desired a breakopen licence, if you are a charitable or religious organization who will be running only one bingo event per week in a community bingo hall and where the prize value at each event is less than $1,000. If your annual total of all prizes are less than $1,000, please refer to the Small Bingo Application.

Class C Restricted Financial Report.pdf
A financial report including the revenues, prizes, expenses and proceeds generated as a result of their bingo events. The Licensee shall complete the financial report on forms prescribed by SLGA and a copy must be submitted to SLGA 30 days following the first six months after the issuance of the Licence and after the expiry of the Licence.

Class C Restricted - Event Close-Out Summary.pdf
A document which details all financial transactions related to a particular bingo event.

Media Bingo Application.pdf
Use this application form to apply for a bingo licence if you are a charitable or religious organization running no more than 3 events per week using mass media communications. This includes, but not limited to radio, newspaper, magazine and television.

Media Bingo - Event Close-Out Summary.pdf
A document which details all financial transactions related to a particular media bingo event.

Media Bingo - Monthly Event Log.pdf
A financial report including the revenues, prizes, expenses and proceeds generated as a result of their media bingo events. The Licensee shall complete the monthly financial report on forms prescribed by SLGA and a copy must be submitted to SLGA by the last day of the following calendar month.

Small Bingo Application.pdf
Use this application form to apply for a bingo licence if you are a charitable or religious organization with an annual total of all prizes under $1000.00.

Small Bingo Financial Report.pdf
A financial report to be submitted to SLGA 30 days following the first six months after the issuance of the Licence and after the expiry of the Licence.

Breakopen

Breakopen Application.pdf
Use this application form to apply for a licence to sell breakopen tickets if you are a charitable or religious organization.

Breakopen Addendum Request.pdf
This form is to be used by a bingo licensee to make changes to any of the information that appears on their bingo licence.

Breakopen - Event Close Out Summary.pdf
A document which details all financial transactions related to a particular breakopen event.

Breakopen Monthly Event Log.pdf
A document showing all breakopen event activity, including the revenues, prizes, expenses and proceeds generated as a result of the breakopen events conducted during the month. The Breakopen Monthly Event Log shall be submitted to SLGA by the last day of the following calendar month.

Raffle

Large Raffle Application.pdf
Use this application form to apply for a raffle licence if you are a charitable or religious organization and the retail prize value is more than $50,000.00.

Large Raffle Financial Report.pdf
A financial report including the revenues, prizes, expenses and proceeds generated as a result of their raffle. The Licensee shall complete the financial report and submit these forms, along with a list of all winners, to SLGA within 60 days of the final draw, or within the time period approved by SLGA.

Regular Raffle Application.pdf
Use this application form to apply for a raffle licence if you are a charitable or religious organization and the total retail prize value for all draws will be greater than $1,000 but less than $50,000.

Regular Raffle Financial Report.pdf
A financial report including the revenues, prizes, expenses and proceeds generated as a result of their raffle. The Licensee shall complete the financial report and submit these forms, along with a list of all winners, to SLGA within 60 days of the final draw, or within the time period approved by SLGA.

Raffle Addendum Request.pdf
This form is to be used by a raffle licensee to make changes to any of the information that appears on the raffle licence.

Small Raffle - Public Amusement Application.pdf
Use this application form to apply for a small raffle licence (prize value not exceeding $1000.00) or a Public Amusement Raffle license (prize value not exceeding 500.00).

Small Raffle Financial Report.pdf
A financial report to be properly completed at the conclusion of the lottery and the signed original forwarded to SLGA within sixty (60) days of the final draw date.

Public Amusement Raffle Financial Report.pdf
For Public Amusement Raffle licencees to be completed after the final draw has been made and submitted to SLGA within 60 days of the final draw date.

Monte Carlo Charity Event

Monte Carlo Charity Event Application.pdf
Application form to obtain a Monte Carlo Charity Event licence from SLGA.

Monte Carlo Charity Event Financial Report.pdf
This report must be filled out after the conclusion of a Monte Carlo Charity Event and submitted to SLGA within 60 days after the event.

Texas Hold’em Poker Tournament

Texas Holdem Poker Tournament Application.pdf
Application form to obtain a Texas Hold'em Poker Tournament licence from SLGA.

Texas Holdem Poker Tournament Financial Report.pdf
This report must be filled out after the conclusion of the Texas Hold'em Poker Tournament and submitted to SLGA within 60 days after the tournament date.

Local Authority

The Saskatchewan Liquor and Gaming Authority may issue a licence pursuant to subsection 207(1)(b) of the Criminal Code, or may designate, in accordance with Section 141 of the Alcohol and Gaming Regulation Act, 1997, a Local Authority to issue the licenses, authorizing charitable or religious organizations to conduct and manage lottery schemes.

Local Authority Raffle Licence Application.pdf
To be used by local authorities for issuing raffle licences with a retail prize value no greater than $1,000.

Local Authority Lottery Financial Report.pdf
To be submitted by licence holders to local authorities 20 days after the final draw date.

Local Authority Statement of Licences.pdf
To be used by local authorities to record all licences issued during the calendar year. This statement must be provide to SLGA for review.
